Studies show that many hospital visits and admissions could be avoided through education and management of heart disease.
Over the past several years, treatment of those with heart failure has undergone tremendous change. Riverside Heart Disease Management uses a multidisciplinary team of health care providers that includes a physician and nurse practitioner, nurses and pharmacists with expertise in heart failure management, a social worker and a dietitian. Working in partnership with you and your physician, Riverside's Heart Disease Management team provides assessment, education, social service support and medical management on an outpatient basis to:
- Decrease your symptoms
- Keep you out of the hospital
- Improve your day-to-day quality of life and the ability to carry out normal activities
- Help you and your family better understand heart failure and how to cope with it

It has been found that those who understand their heart conditions and participate in their treatment plans have an improved quality of life and make better decisions about their healthcare. Since opening the program in December 1997, patients have received care with the following outcomes:
- 60% reduction in heart failure admissions
- 36% reduction in non-heart admissions
